  • Overview

Dr. Robert Mentz examines the role of vericiguat in ambulatory patients with symptomatic HFrEF and LVEF <45% who have persistent symptoms and residual cardiovascular risk despite optimized foundational medical therapy. He reviews the 2026 ESC guideline recommendation that vericiguat may be considered to reduce the risk of heart failure hospitalization and cardiovascular death, including its Class IIb, Level B1 designation and positioning as additional rather than foundational therapy. Dr. Mentz also compares the ESC recommendation with the 2024 Chinese heart failure guideline and 2026 iCARDIO guideline, highlighting differences in recommendations and patient selection. The program reviews pooled evidence from VICTOR and VICTORIA and its application to stable ambulatory patients and concludes with practical considerations for when and how to incorporate vericiguat in clinical practice.
